4 Things Keeping Insurance Leaders Awake at Night

Private health insurers are facing one of the most complex operating environments in decades. Cost pressures are rising, member expectations are shifting, and digital transformation has moved from aspiration to obligation. In short, the sector is being reshaped by forces it cannot ignore.

As part of the early engagement for the 2025 PHI Innovation Report, leaders who pre-registered were asked a simple but powerful question: Which drivers do you think will have the biggest impact on PHI over the next decade?

Their responses revealed a striking pattern. Four forces dominated: disease, digital, demographics and dollars.

Eighty-five percent of respondents identified chronic and preventable disease as the single most influential factor shaping the future of PHI. Seventy-five percent highlighted digital and technology integration as essential to competitiveness and efficiency, while seven in ten pointed to affordability and price sensitivity as a growing risk to membership. Sixty percent cited Australia’s ageing population and aged care as a structural challenge the sector can no longer delay addressing.

Mental health, customer experience, and sustainability were also recurring themes, reflecting an industry that sees health, value, and trust as inseparable parts of the same equation.


1. Chronic and Preventable Disease

Leaders are unanimous that the health of Australians is the system’s greatest test. Chronic conditions drive most claims costs and are rising fast. Insurers are increasingly aware that prevention is not optional; it is the only sustainable path forward. Expect more funds to invest in early intervention, digital monitoring, and health-at-home programs that keep people well for longer.

2. Digital and Technology Integration

Digital transformation is no longer a differentiator; it is survival. From predictive analytics to virtual care and automated claims, technology is now the foundation for efficiency, experience, and innovation. The challenge is not just building digital tools but embedding them into workflows, culture, and member interactions so they deliver measurable value.

3. Affordability and Price Sensitivity

Even the best innovations mean little if members cannot afford to stay insured. Rising living costs and stagnant wages are forcing Australians to reconsider the value of their cover. Funds that can prove tangible value through smarter care models, transparent pricing, and community alignment will be the ones that retain trust and membership.

4. Ageing Population and Aged Care

Australia’s ageing population is both a demographic certainty and an economic challenge. More members will need complex, coordinated care that bridges hospital, home, and community. Insurers who build strong partnerships across these settings will be best placed to navigate the costs and opportunities of an older population.


Beyond these core themes, leaders also pointed to mental health, customer experience, and system sustainability as emerging priorities. These reflect a broader shift, with health funds beginning to see their role not just as payers of claims but as active participants in shaping the future of care.


The bottom line

The decade ahead will be defined by how insurers respond to these four forces. Those who take a proactive approach, tackling disease, leveraging digital, protecting affordability, and preparing for demographic change, will be the ones who thrive. Those who do not will find the market, and their members, moving on without them.
